Report a lost or stolen ID Badge

Lost or stolen ID cards should be reported to the IT Support Office where a replacement card can be obtained. There is a $30.00 replacement fee, payable by debit or credit card only, for lost or stolen ID cards. The replacement fee is waived upon the presentation of a police report.
